Know about First Pass or Pre-systemic Metabolism



ntroduction: This refers to metabolism of a drug during its passage from the site of absorption into the systemic circulation. All orally administered drugs are exposed to drug metabolizing enzymes in the intestinal wall and liver (where they first reach through the portal vein).

What does First Pass metabolism mean?

First pass metabolism or pre-systemic metabolism also known as first pass effect is the case of drug metabolism whereby the concentration of the drug specially when it is administered orally it is greatly reduced before they reach the systemic circulation.

It is the fraction of drug lost during the process of absorption which is generally related to the liver and gut walls. Some of the drugs which experience the first pass effect or pre-systemic metabolism are imipramine, morphine, buprenorphine, diazepam, midazolam, pethidine, tetrahydrocannabinol, propranolol, cimetidine, lidocaine and nitroglycerin.
